Chevy Chase is
out of rehab — with the former "Saturday Night Live" star reliving old glories while checking out of the Hazelden Addiction Treatment Center in Minnesota. The slimmed-down star proved he could fit into a pullover from his 1985 movie "Spies Like Us" while heading home with his wife Jayni after risking his life with booze binges!

Insiders told
The National ENQUIRER that Chevy — who weighed 358 lbs. when packed off to rehab in September — desperately needed treatment after a series of psychotic rampages fueled by alcohol. His handlers had downplayed the trip to Hazelden as a "tune-up," but sources said it was a bid to save his life!

[LANGUAGE WARNING] As seen in this video, Chevy didn't have much patience for reporters checking in on his welfare, either. The comic may have rested up in rehab, but he's as hot-headed as ever — with insiders saying that he had a host of problems that needed to be addressed....

“His
heart attacks, massive
weight swings and violent moods have crushed his career
and his health,” declared a source close to Chevy (seen here in 2014). “He needs to do something now … or it will be over in more ways than one!”

The rehab came just a year and a half after Chevy’s lifestyle excesses led to an emergency room visit for heart issues — as he piled on a gut-busting 388 pounds! The comic had laughed off his previous 1986 stint at the Betty Ford Clinic in a 2010 interview, saying, “I was pretty low-level when it came to drug abuse.”

But there was little to laugh about as Chevy's drinking and nasty mood swings kept alienating Hollywood against him! The cranky comic’s foul moods and disposition continued to have a self-destructive impact on his career — with his costars
mocking the comedy legend after he was finally fired from the popular sitcom "Community" in 2014.

Insides said that Chevy's second try at rehab was also his last chance for a showbiz career. “No one wants to hire him and he’s desperate and alone,” says the source. “He figures this is his Hail Mary pass … his last chance to get back on track and save his career … and his life!” But some medical experts have their concerns about Chevy's future. “It’s unfortunate that his PR team called it a ‘tune-up,’” warned rehab expert Ben Levenson of The Levenson Foundation and Origins Behavioral HealthCare, who has not treated Chevy. “People don’t say, ‘I’m just getting a little chemo for my cancer.’ They say, ‘I’m in the fight of my life.’ That applies to addiction.”

The fizzled funnyman has already spent a long career battling booze, drugs and his co-stars — going back to his "Saturday Night Live" days when his career was skyrocketing. What should have been a triumphant return to the show as a host in 1978 ended in a backstage fistfight with
Bill Murray. “Chevy," said the insider, "was a rising star who shot himself down!”
