After suffering a relapse, troubled actor Tom Sizemore has secretly checked into rehab for hardcore drug abuse, The National ENQUIRER has learned exclusively.

The “Saving Private Ryan” star, 53, has claimed he’s been on the wagon since 2009, and insisted that a highly publicized video showing him getting high was filmed before he got clean.

But, said a source, “Tom was living a lie. He was telling everyone that he was clean and sober, when in reality, he had relapsed again.”

The ENQUIRER exposed Tom’s latest drug troubles in January when we published shocking still photos from a video showing the actor getting high on heroin and crystal meth in late 2013.

Tom later claimed the footage was filmed in April 2009, before he appeared on “Celebrity Rehab with Dr. Drew” and kicked his substance abuse issues.

“Those are the choices I made back then and they are coming back to haunt me,” he said in an interview.

However The ENQUIRER has learned that Tom talked about his therapy on the tape – which means he flat-out lied about when it was filmed!

“Dr. Drew said that sex problems are because of the Internet,” Tom – who has twin boys, Jagger and Jaydon, 9, with ex Janelle McIntire – told a druggie buddy on the tape. “Mine’s compounded because I grew up in a whorehouse … (Dr. Drew) told me that I have an entrenched sex addiction. He thinks the only thing that’s going to cure it is old age.”

But most damning of all, the actor and his pal discussed his finances in 2012 and 2013 – several years after Tom claims the tape was made!

“Don’t be fooled that those close to Tom were oblivious to his shenanigans. He was NOT sober for five years. The video prompted an intervention, and thankfully he got the help he needed,” added the source.

When contacted, a rep for the actor confirmed he’d entered a facility for treatment.

At the time, Tom – one of the most sought-after supporting actors of the ’80s and ’90s, starring in “True Romance,” “Natural Born Killers” and “Heat” – was spending up to $700 a week on drugs, according to the source.

Last week, he returned to the public eye, even shopping for rings for a rumored engagement with his young girlfriend in L.A. while showing off his Narcotics Anonymous chip on his keychain.
