Rhinestone Cowboy singer Glen Campbell has taken his last ride!

Sources told The National ENQUIRER that the 79-year-old Alzheimer’s disease–ravaged star was removed from a care facility by his fourth wife, Kim, and taken to their Nashville area home to live out his final days.

For the maker of such hits as “Gentle on My Mind” and “Wichita Lineman,” a source said: “Sadly, it looks like the end of the line for Glen.

“Kim took him home because she frankly doesn’t know how much longer she’ll have left with him!”

In June, The ENQUIRER exclusively reported that Glen can no longer speak with the exception of mumbling “I love you.”

Glen was diagnosed with the debilitating disease in 2011, and it’s since progressed to the seventh and final stage, which sometimes causes him to lash out in fear.

Kim acknowledged: “He punched me in the eye and I had a black eye for two weeks. It’s really hard to cope with. I know that’s not him — it’s just the Alzheimer’s.”

Two of Glen’s oldest children — Debby Campbell-Cloyd, 58, Glen’s daughter from his first wife, and Travis Campbell, 49, Glen’s son from his second wife — had filed a petition in January to gain control of their dad’s medical and financial decisions.

In April, the older children reached a tentative deal with their stepmother in which they were allowed to see Glen twice a month.
